Medication Management Tips

Often seniors take multiple medications daily that in some cases have been prescribed by a couple of different doctors for different conditions.  In addition they may also be taking over-the-counter (OTC) medications.  All of this can lead to problems with drug interactions and side effects that range from mild to severe including death.

In a report by the US News & World Report, it cites statistics from the Centers for Disease Control and Prevention (CDC) that suggest each year there are four visits to the ER per 1,000 adults attributed to adverse drug effects.  First published in the Journal of the American Medical Association, statistics find that almost 60 percent of Americans take at “least one prescription drug, and nearly 20 percent are taking more than five prescriptions medicines.”  The study also looked at OTC medications and warns that there is a huge potential for “side effects, drug-drug interactions and negative outcomes.”  It also states that for the elderly, the possibility of dangerous drug interactions is even greater. 

One of the most important things that a patient or family member can do to reduce the likelihood of this happening is to communicate with all of your health care providers about the prescription and OTC medications that you take.   It is a good idea, especially if you take more than one medication, to write down the names of the drugs and the amount you take each day and carry it in your wallet.  You can also take a picture of the list of medications with your cell phone so you always have it handy.  Another tip is to fill your prescriptions at the same drug store and to build a relationship with the pharmacist as he or she can help to spot potential problems involving drug interactions.   

Annually review the need for each medication with your health care provider and discuss how long you will need to take the drug.  Lastly be sure to check each new prescription container for accuracy as pharmacies sometimes do make mistakes.

What is Congestive Heart Failure?


According to the American Heart Association, heart failure, also known as congestive heart failure, is one of the most common causes of hospitalization for adults age 65 years and older.  Congestive heart failure occurs when the heart does not pump blood as efficiently as it should and therefore is not able to meet the body’s blood and oxygen needs.  Since the amount of blood pumped is not adequate to circulate the blood returning to the heart from the body and lung, it causes fluid (mostly water) to leak from capillary blood vessels.  If left untreated, the condition can worsen. 

While symptoms of the condition can vary, common signs of congestive heart failure include fatigue and weakness, a reduced ability to exercise, swelling (edema) and shortness of breath.  The disease is commonly caused by coronary artery disease, high blood pressure, disorders of the heart valves, and other problems.  Less common causes include viruses or thyroid disorders. 

If a person already has heart disease, certain medications such as nonsteroidal anti-inflammatory drugs (NSAIDs) and calcium channel blockers can also cause the condition to develop or become worse.   Also having diabetes as well as taking some of the drugs used to treat type 2 diabetes can increase the risk of heart failure in some people.   

The Mayo Clinic explains that, “heart failure can involve the left side (left ventricle), right side (right ventricle) or both sides of your heart. Generally, heart failure begins with the left side, specifically the left ventricle — your heart's main pumping chamber.”  The disease is diagnosed by a thorough physical exam, a series of laboratory tests, and a patient’s medical history.  If you have the disease, you must carefully follow their doctor’s orders.  Treatment includes reducing sodium in your diet, lifestyle changes such as losing weight and managing stress, medications, mechanical therapies, surgery or a heart transplant.

Good Oral Hygiene is Important for Seniors

Good oral hygiene is an essential part of maintaining one’s overall health and quality of life.  Even though seniors are living longer, it does not mean that they cannot keep their natural teeth.   Brushing at least twice daily, flossing, and regular visits to your dentist can prevent problems from occurring. 

Seniors are at a greater risk for developing periodontal disease which is the inflammation of the gums caused by bacteria or plaque that forms on the teeth.  It is very common and one of the leading causes of tooth loss.  Signs of periodontal disease are bleeding gums, receding gums, bad breath, and shifting or loose teeth.  There is evidence to suggest that periodontal disease is linked to heart disease, stroke, and perhaps increases the complications of diabetes.  

Dry mouth is another problem faced by some seniors. It can be caused by over 400 commonly prescribed medications such as those for high blood pressure, heart problems, and depression to name just a few, and also from radiation or chemotherapy treatments.  The danger with dry mouth is that it increases the chance of tooth decay or gum disease.  You need to take extra care that you are drinking plenty of water and stay away from caffeine, tobacco, alcohol, and sugary foods.  There are some over-the-counter medications that can help relieve the symptoms of dry mouth.

If you do have dentures, be sure to take good care of them as they will last longer.  The average life of dentures is five to seven years and it is important to follow good cleaning procedures.  Brush your dentures daily with mild soap or denture paste, and if you are not going to wear them at night, soak them in water or denture cleaning solution (be sure to rinse them thoroughly afterward). 

Receding gums caused by periodontal disease or the result of aging can increase the chance of tooth decay occurring at the gum line which causes serious problems to the roots.  Good dental care is the best tool in preventing further damage.  See your dentist at the first sign of any bleeding or pain in your mouth.

Cataracts are Common in Seniors

As we age, it is more important than ever to get regular eye exams.  The American Optometric Association recommends that every person over the age of 60 have an annual eye examination.  They also advise that if you notice any changes in your vision, see an eye doctor immediately. 

Cataracts are a common eye problem in older adults and develop as the tissue in the eye’s lens breaks down and clumps together causing cloudy or opaque areas.  Aging or an eye injury can cause cataracts as well as other conditions such as diabetes and the long-term use of steroid medications.

The size and location of the cataract determines how much it interferes with your vision.  If the cataract is only affecting a small part of the eye, you may not even know you have one.  Noticeable symptoms occur as the cataract grows and starts clouding more of the lens and distorting the way light passes through the lens.
 
The Mayo Clinic says that cataract symptoms include:

·         Clouded, blurred or dim vision
·         Increasing difficulty with vision at night
·         Sensitivity to light and glare
·         Need for brighter light for reading and other activities
·         Seeing "halos" around lights
·         Frequent changes in eyeglass or contact lens prescription
·         Fading or yellowing of colors
·         Double vision in a single eye

Once a person cannot clear their vision with prescription glasses and the cataract is interfering with normal daily activities including driving at night and reading, surgery is the only effect treatment.  The procedure involves removing the damaged lens and replacing it with an artificial lens called an intraocular lens, which becomes a permanent part of the eye.

The surgery is done on an outpatient basis and the person experiences a few days of discomfort.  The complete healing process takes about eight weeks.  If a patient needs surgery on both eyes, the second one is done once the first eye has healed.

Visiting a Loved One with Memory Loss

The Reflections Neighborhood at MorningStar of Glendale senior living community emphatically agrees with the Alzheimer’s Association about the importance of visiting your loved one in a memory care community.  While it can be hard to see your parent or family member struggling with memory loss, many times they are aware that they have a visitor even if they do not know your name. 

The Alzheimer’s Association wants you to remember that you are an important link to your loved one’s identity and to keep in mind that there is more to them than just their memory.  They recommend keeping visits simple and to try to limit distractions and excess noise.  In addition they remind visitors that they may have to adjust their expectations and to not take it personally if your loved one becomes angry or upset with you.  It is also helpful to know as much about the disease as you can as it will help you to better understand the struggles of memory loss.

Jolene Brackey also shares some tips from her book, Creating Moments of Joy for the Person with Alzheimer’s Disease, to help family members when visiting.  She suggests you begin your visit by introducing yourself, and to use their given name rather than calling them “mom” or “dad” as they may have forgotten that their kids are already grown.  If your loved one has a familiar nickname, you can use that.   

Ms. Brackey also recommends that you visit “one on one” or in small groups, and to bring the conversation around to pleasant memories from their past.  If you do get them talking, give them plenty of time to respond to a question or to follow directions.

Time to Consider Senior Living?


Often a family is not sure whether it would be better for mom or dad to move into a senior living community.  Making the decision more difficult is the fact that many seniors can be resistant to moving, and they do not like the idea of leaving their home and everything familiar to them.  However the most important thing to consider is whether your parent’s current living situation meets their overall health and medical needs, their physical needs, and safety.  If it doesn’t, it is time to start a conversation about making a change.

Memory Care at MorningStar of Beaverton
